本研究以活動理論觀點探討醫院雲端服務創新模式,活動理論係架構於主體、工具、客體、規則、社群及分工等六元素,透過蒐集個案1990~2013 年之次級資料共2322筆,再由資料判讀後獲得109件雲端服務創新專案,進而以活動理論之角色為分析單元,最後獲得349個單元事件。本研究採內容分析法,並透過深度訪談進一步發掘部分結果的深度意涵,最後獲得四項研究發現:(1)健康雲較忽略客體元素,照顧雲較重視社群元素,而醫療雲之規則元素比例偏低;(2)主體較重視醫院對民眾之服務提供及社群協同合作之服務設計,而較忽略民眾對醫院之服務需求;(3)本模式具有六種驅動類型,以社群驅動為主,客體驅動最少;(4)本模式具主從式結構,並以社群驅動之從式結構為主,主體與社群須協同研發與品質稽核,以強化內外控機制。

This study explores the innovation model of hospital cloud service (HCS) based on the perspective of activity theory. The activity theory consists of six elements concerning subject, tool, object, rule, community, and division of labor. Through collecting the secondary data of the case from 1990 to 2013, we originally obtained 2322 events. However, only 109 projects are related to the service innovations provided by the case through screening the originally collected data. Furthermore, we used each role of the activity theory as an unit of analysis and finally obtained 439 units of analysis. This study adopts method of content analysis and further seeks the insights of part of the results through in-depth interviews. Finally this study proposes the following four findings. First, health cloud service pays little attention to object element and care cloud service emphasizes community element. The rate of rule element in medical cloud service is lowest among the three cloud service systems. Second, subject element emphasizes the hospital-to-patient service provision and the service collaboration design with community. However, subject element places less emphasis on patient-to-hospital service requirements. Third, there are six driven models existing in HCS innovation, where community element plays key driver role and objective element is the least. Fourth, HCS innovation model is a master-slave type and the slave model driven by community is the key. In addition, the model emphasizes the collaboration development and quality control by subject element and community element in order to enhance organizational control mechanism.
